Official High School Transcript (with GPA) or copy of current GPA and a copy of acceptance to a qualified institution or school registration form.
Attach extracurricular activities/community service activities or other volunteer activities you have participated in within the past two years. (For example: Organization Activity Dates) Attach recommendation from teacher/counselor/supervisor.
Academic Honors received: Attach additional sheets of paper if more space is needed.

I agree that the Baltimore County Employees Federal Credit Union may use my name and/or photo for publicity purposes if I am selected as a scholarship recipient. Please note: Award winners will be notified by June 30, 2018. Award checks are made payable to the recipient and the educational institution being attended.
